Healthy Ageing Hub Advisor 

Are you passionate about making a difference to the healthy and wellbeing of older people living in the community? Do you thrive working in a values based team and establishing new programs? 

Join JNC’s dynamic and trusted team to establish and operate this exciting new service. The new Eastern Subrubs Healthy Ageing Hub will promote positive ageing and social connection, support primary healthy care through provision of social care and assist older people with navigating health, aged care, social and community services to meet their needs. 

Aged Service Coordinator 

As an Aged Services Coordinator you’ll support older people accessing support through the Commonwealth Home Support Program. You will assess needs, create care plans and service plans and roster services on a weekly basis. You will work closely with Social Support Workers and volunteers to ensure we provide person centred and inclusive services. You will promote our services at local expos and community events. 

Casual Social Support Workers

Are you keen to support older people to feel a part of their community and to build your skills in a fast-growing industry?  If the answer is yes, then please consider applying for a position of Social Support Worker with our aged services team.
